Medicina is recalling its IV Luer Slip syringe due to incorrect packaging.
According to the company, syringes were incorrectly packaged with a needle, which means they are not sterile and could cause a needle stick injury. The device affected by the recall is 3ml Luer Slip syringe without needle with Medicina product code IVS03, NHS supply chain code FWC346, batch number 19040303, with a manufacture date of April 19 and an expiration date of April 2024.
Medicina recommends to identify and quarantine affected devices in stock and complete the “FSCA Acknowledgement Form” in the FSN, even if the healthcare professional doesn’t have affected devices left in stock. The company also recommends reporting suspected or actual adverse events involving the devices through a local incident reporting system.
